DS-160 Visa Application Form Required at Listed Embassies and Consulates
A new Visa Application form (form DS-160) will be gradually phased in, and will ultimately replace forms DS-156, 157, 158, and other related Visa Application forms, such as the DS-156E, 156K, and 156V. The new form can be prepared on-line and submitted to the U.S. State Department electronically over the Internet, including the ability to attach an electronic passport photo.
